MyChart is your secure online health connection, anytime and anywhere. As SickKids’ online patient portal, it’s the easiest and most convenient way to receive care and communicate with your health team using a mobile phone or computer. Use MyChart to join virtual visits, stay in touch with your care team, and to see most of your health information.

  • View upcoming appointments and receive appointment reminders
  • View and update medications, allergies, and personal information

Frequently asked questions

SickKids patients can sign up for their own MyChart account. Patients can decide to also grant access to their parents or legal guardians. This is known as proxy access.

To apply for MyChart access, please fill out the appropriate MyChart Access Request Form. The Health Information Management team will review your application. If we approve your application, we'll send you an email with your activation code and instructions on how to login.

It is our priority to ensure that your personal health information is kept private and secure. Every user controls his or her own username and password. We also use an encrypted connection. Your information is not stored online, but rather on a secure server.

Patients over the age of 12 will be able to sign up for their own MyChart account.

Patients over the age of 12 with MyChart access can login and share their record with someone else. This is known as proxy access.

For patients under the age of 12, only parents or legal guardians can be granted proxy access.

Information in your MyChart can be printed as needed and shared. For example, you may want to print your medication list, or lab results and share with your family doctor.

A proxy is a parent, caregiver or guardian who has access to your health record. Proxies see everything a patient can see in MyChart. Once you have your own MyChart account, it's up to you who has access to your health information. You can control proxy access by adding or removing proxies by following the steps below.

To add a new proxy:

  1. From the MyChart menu, click Share My Record and select Friends and Family Access.
  2. Click Invite Someone to give proxy access to a parent or caregiver. They will receive an email notification inviting them to access your record. You can revoke their access at any time.

To remove an existing proxy:

  1. From the MyChart menu, click Share My Record and select Friends and Family Access.
  2. Click Revoke to remove someone's proxy access. They will not receive a notification when you do this.

If you already have a MyChart account, visit the MyChart website to log in.

To access your entire health record, you will need to submit a request to Health Information Management (formerly Health Records). The request process remains the same.  

A written request form will need to be filled out and mailed or emailed to Health Information Management. Now, instead of picking up in person, or waiting for the information to be mailed, patients can choose to receive their record via MyChart in a password protected PDF file. 

If you're looking to deactivate your MyChart account, follow these steps:

  1. From the MyChart menu, click on Security Settings.
  2. Follow the link in the Deactivate Your Account box to visit the account deactivation page. 
  3. Please read all the instructions on this page before deactivating your account.
